Additional questions
Additional questions are your own questions, asked at the point someone signs up. You build them as a form, then attach that form to a session, a session group, a lead form, or your staff application.
Assigning a form does something else: an assigned form waits in the parent or staff portal to be completed in someone's own time. You may hear additional questions called intake forms. It's the same feature.
Where you can add them
Session - during enrollment, for each camper on that session
Session group - during enrollment when the group is picked, separate from any questions on the sessions inside it
Camper lead form - after the family enters their camper details
Staff lead form - after the contact and eligibility fields
Staff application - near the end, before the resume step
Sessions and session groups are on your season's Sessions page. Lead forms and the staff application settings are in your season settings.
Adding questions
Open the record and find Additional questions:
Add questions - build a new form, attached here alone
Use an existing form - reuse one you've published
Copy from a form - fork a published form, leaving the original alone
Once attached, the same menu offers Edit questions, View responses, and Remove questions. Removing unlinks the form. Answers stay on record.
Publish before anyone can see them. A form built from a session starts as a draft, like any other form.
Which forms you can reuse
Use an existing form lists published forms for that audience assigned to Everyone, plus forms segmented to the session you're editing. Segment and individual forms are left out. Copy from a form takes any published form.
A form used in more than one place is marked Shared, and editing it changes every place at once. Copy it instead if two sessions need to ask different things.
Camper forms cover sessions, session groups, and camper lead forms. Staff forms cover staff lead forms and the staff application.
What people see
During enrollment, the questions are a Forms step after extras and before review, one form at a time for each camper. Answers save as a draft, and a form the camper already completed this season is skipped. A form scoped per family is asked once for the family.
On lead forms and staff applications they're a step in the same flow, answered before the person can submit.
Where the answers go
View responses on the Additional questions menu shows every submission for that form
Enrollment answers land on the camper's enrollment Forms tab, per-family answers on the family, and staff application answers on the employment. Lead answers move across when you convert the lead
Forms attached this way aren't in the Forms section. Reach them from the record they're attached to. Detach one from everywhere and it moves back into that list under Individual
Reporting on the answers
Every published camper form adds a column per question to your Campers table, plus status and overdue columns, grouped under the form's name in the Columns menu. They sort, filter, and export like any other column. Staff forms do the same on your staff table.
Headings, paragraphs, separators, file uploads, and signatures get no column.
Which to use
Ask as additional questions when the answer depends on what someone picked, or when you need it before they finish. Assign a form when it can wait, like medical history or travel plans. Both land on the same Forms tab.
